Documents: 鈥淯nsaved changes鈥� error when adding a form to a document

When adding a form to a document, an 鈥淯nsaved changes鈥� error appears that cannot be closed and prevents interaction with the application.

Workaround

Refreshing the browser page removes the error message and allows the application to be used again.
